Every player needs to sign a waiver form before they start playing. Please have a valid government-issued photo ID at the time for the waiver signing. If you are under the age of 18, a waiver form must be signed by your parent or legal guardian (a person to whom is given the legal title to entrust another’s benefit). It cannot be signed by an older sibling, friends of legal age, uncles, aunts, or friend’s parents, etc. We accept different forms of ID from drivers licenses, liquor commission card, passport, a combination of a student card (with photo) or other photo ID along with a medical card or birth certificate.

If you’re signing for a player who is a minor, but won’t be there for the day of the event to sign the waiver form. You can come in anytime before the event to sign the waiver form. The waiver form only needs to be signed once, it’s not necessary to bring your parent or legal guardian every time you play. We highly recommend letting them know every time you plan on playing a game.
